Husbands go off to work…children go off to school…wives go about their daily routine…everyone, everyday, is moving in a defined path.

Life is fragile and can be taken at any time, with or without notice. “See you tonight,” may never happen…plans for tomorrow may never come to pass.

On any given day…make it shine. Take no day for granted.

Advance Directives

Clicking on the link at the bottom of this post will open a page with a form that you may print out. The form is for stating your advance directives in the state of Florida.

These advance directives basically allow you to write out now, what you would like to be done if you are unable to communicate, such as if you fell into a coma.

These forms are not a substitute for legal advice, but you can give them to staff at your doctors’ offices now and they should be able to tell you whether or not the forms will agree with current laws.

Whenever you enter a hospital, nursing home, or rehab facility, you or your healthcare surrogate (someone whom you appoint to carry out your wishes or make decisions on matters that you haven’t spelled out elsewhere) can ask that this form be added to your file.

The form requires two witnesses, but doesn’t need to be notarized.
